| Adult Day Care
is a service that provides an organized program of daytime services in a community group
setting. An adult day care supports the personal independence of older adults and promotes
their social, physical and emotional well-being. |

| Care Management services assist older
adults with complex care needs by identifying, accessing and coordinating services that
are necessary to enable the individual to remain in the least restrictive environment. |
| ALL LOCAL DEPARTMENTS OF SOCIAL
SERVICES PROVIDE CARE MANAGEMENT SERVICES IN VARYING DEGREES. Some assist with services
provided within the agency while others will provide whatever assistance is needed to
identify and access the required services. |
| COMMUNITY
ALTERNATIVES PROGRAM FOR DISABLED ADULTS |
| The Community Alternatives Program for
Disabled Adults (CAP-DA) is a program specifically for those individuals with limited
financial resources who need assistance to stay in the home. |
| The Senior Community Service Employment
Program is a program designed to provide subsidized employment for low-income older
adults in order to provide training to obtain full time employment. |
| The Workforce Investment Act (WIA) assists adults, including those age 55 and over, to find employment through either
counseling, job search, basic education, on-the-job training, individual referral or work
experience. |

| ENERGY ASSISTANCE
PROGRAM |
| Energy Assistance Program is available on an annual
basis for those individuals who qualify. Generally late summer or early fall is the
sign-up period for this service that provides financial assistance to pay heating bills. |
| The Ethics Committee is an interdisciplinary group
of professionals trained in medical ethics to review the ethical aspects of care and
treatment for residents of long term care facilities. |
| Ombudsman Program provides support for
family members/residents in long term care facilities, encourages the
development of individual facility family councils and facilitates the
regional family council. |
| Food Stamps provide food assistance for low-income
individuals. |
| Health Screening is a service that consists of
evaluating personal and family medical history information and testing for diseases or
health problems. Health Screening can detect problems, abnormalities, and/or identify
factors that may make a person at risk for a condition or problem. |

| Home Health is the provision of skilled health care
in the home that is prescribed by a physician. Allowable activities include: skilled
nursing (both registered and licenses practical nurses); physical therapy, speech therapy
and occupational therapy; medical and social services as well as nutritional care
services. |

| Independent Living refers to several alternatives
that enable an older adult to live independently in a group setting. |
| Assisted Living residence means any group housing
and services program for two or more adults, by whatever name it is called, which makes
available at a minimum, one meal a day and housekeeping services and provides personal
care services directly or through a formal written agreement with one or more licensed
home health agencies. |

Homes, provide extensive care to people of all ages needing medical services. |

| INFORMATION AND CASE
ASSISTANCE |
| Information and Case Assistance identify critical
services that assist older adults, their families and others acting on behalf of older
adults, in their efforts to acquire information about programs and services and to obtain
appropriate services to meet their needs. |
| In-Home Aide Services are those paraprofessional
services that assist the individual and/or family with essential home management, personal
care tasks, and/or supervision necessary to enable the individual and/or family to remain,
and function effectively, at home as long as possible. |
| Institutional Respite Care Service is temporary
placement of an individual who requires constant care and/or supervision out of his/her
home to provide the primary, unpaid caregiver temporary relief from care giving
responsibilities. |

| Legal Services provide older adults with
information on their legal rights, legal advice, legal benefits, entitlements, and/or
appeals when referral to a human service agency or a pro bono service is inappropriate,
and community education. |

OMBUDSMAN (Advocate) |
| An Ombudsman is legislatively mandated to serve as
an advocate for residents in Assisted Living and Nursing Facilities. The focus of the
Ombudsman program is to enhance the quality of life for residents. Community Advisory
Committees serve as local Ombudsman. |
| Congregate Nutrition Services promote the health
and well-being of older adults through the provision of a nutritious meal five or more
days a week. |
| Home Delivered Meals help maintain and improve the
health of impaired home-bound older persons by providing nutritionally balanced meals
served in the home. |
| The Supplemental Meals Program is a service that
provides a meal composed of a nutritional supplemental product. The meal is provided to
persons who are at nutritional risk and the service must be authorized by the individual's
physician. |
| Medicaid provides financial assistance for health
care needs. While low income older adults may qualify for this financial medical
assistance while living independently at home, the majority of older adults using this
service are residents in Nursing Homes. |
| A Multipurpose Senior Center provides a broad
program of services and activities which must include but not be limited to: outreach,
information and case assistance, health promotion, social, nutritional, and educational
services, volunteer opportunities, and recreational activities for older adults 60 years
of age and older and their spouses. |

| Senior Games is a health and wellness program that is a
network of local, state and national competitions in a variety of sports and arts events.
An individual must be 55 or older to participate. |
| The Transportation Service provides travel to
and/or from community resources such as medical care facilities, nutrition sites, or other
locations for the purpose of accessing needed services or carrying out activities
necessary for daily living. |
There are several opportunities for individuals to
volunteer through the Centralina Area Agency on Aging.
The Region F Aging Advisory Committee comprises members from each of the eight
counties in Region F. Individuals are appointed by the county Commissioners in each
county. The responsibilities are to attend four meetings each year in Charlotte; to learn
about the aging network in the respective county; and to make recommendations to the
Centralina Council of Governments Executive Committee and Board regarding initiatives and
funding allocations on behalf of older adults in the eight county region. For more

| The Adult Care and Nursing Home Community
Advisory Committees in each county comprise volunteers who serve as advocates for
residents who live in Assisted Living and Nursing Homes. Committee members visit
facilities, as a group, on a regular basis to monitor for resident's rights and speak out
for quality of care for frail citizens under the supervision of the Centralina Ombudsmen.
Mandated responsibilities require approximately eight (8) hours per month. For more

| The North Carolina Senior Tar Heel Legislature
is a statewide organization consisting of a delegate and alternate from each county in
North Carolina. The purpose of the organization is to bring to the attention of the NC
General Assembly issues that impact the well-being of older adults in North Carolina.
These individuals are elected by the Region F Aging Advisory Committee. The
responsibilities for this volunteer position are: attend four meetings each year in
Charlotte; attend three meetings each year in the Raleigh area; get to know the state and
national elected officials with the area; become knowledgeable about issues and concerns of
older adults in the county; and be willing to advocate with the NC General Assembly and US
Congress about issues identified by the NC Senior Tar Heel Legislature. Anyone age 60 or
older may be nominated by contacting the Area Agency on Aging at 704-372-2416 for
information or a nomination form. |
